The National Health Care Anti-Fraud Association reports that four billion health insurance claims are processed in the US each year.

Though a small portion of health insurance claims across the nation are ultimately found to be fraudulent, because of the high number of insurance claims overall, those that are indeed fraudulent amount to a large price tag for consumers and the industry alike.

As more affordable health insurance options have made their way to the marketplace over the past 10 years, the number of fraudulent claims have increased. One practice that is seen quite frequently is the insertion of false diagnoses by practitioners looking to bill insurers for these claims.
